Please bring all the necessary documentation including:

1. Identification Current Drivers License, State ID or Government Issued Residency Documentation
2. Social Security Cards Valid, signed Social Security Cards for yourself and all persons in your household who are age 18 or older
3. Utility Bills A copy of your most recent & active utility bills from each utility supplier (both gas and electric)
4. Income Proof of ALL earned and unearned gross income received by ALL household members in the month prior to the month of your application
5. Paid Medical & Dental Expenses Proof of medical/dental expenses you paid out during the month prior to the month of your appointment or application date.
6. Child Support &/or Alimony Proof of any child support and/or alimony you paid out or received in the month prior to the month of your appointment.
7. Proof of Disability Must be disabled for at least 6 months. Proof: Letter of Social Security Benefits.
8. Proof of Age for any Child in the Household Age 5 and under. Birth Certificate, Blessing Certificate, Immunization Record, etc.
